IMPORTANT! Each camper is held to provide a good-health and sport activity fitness certificate.

PARENT¹S CONSENT - 1) I hereby consent to my daughter¹s/son¹s participation in all Camp activities. My daughter/son has recently been examined by a doctor and therefore she/he is fit for any Camp activity.
	2) Should it be impossible for you to contact me in case of an emergency, I hereby authorise the hospitalisation and required medical treatment as prescribed by the Camp doctor.
	3) I also authorise my daughter¹s/son¹s assistance and care by Camp doctors and physiotherapists.
	4) I hereby waive and release the WBSC SuperCamp staff from any and all liability for any expense, compensation of damage, loss of documents or accidents incurred or caused by my daughter/son.
	5) I hereby declare to be acquainted with the conditions of the Camp insurance policy and to accept them and that by enrolling my daughter/son I waive any claim for damages to any amount greater than that covered by the insurance.
	6) I hereby agree that the competent court for any dispute or controversy shall be that of Forlì.
